Course overview

This module will provide the opportunity for students to synthesise the knowledge and skills required  to work effectively with those living with a long term condition, and the management strategies that  can be employed at each stage of the patient journey.

Indicative Content 

  • Long term conditions in context.
  • Pathophysiology, symptomology, care and management of a variety of long term conditions e.g. respiratory disease, cardiac disease, diabetes, dementia, frailty.
  • Psychosocial impact of living with a long term condition. Self-management in long term conditions.
  • Primary and secondary prevention Managing complex care
  • Long term conditions and mental health.
  • Empowerment
  • End of life care and Palliative care, LPA, DNAR’s and advance care planning
  • Ethical considerations in long term conditions
  • Multi morbidity, and poly pharmacy and prescribing decisions
  • Structured medication reviews


Learning Outcomes

  1. Critically apply knowledge and understanding of the pathophysiology, symptomology, care and management of long term conditions.
  2. Synthesise evidence related to the physical, social and psychological impact of living with a long term condition.
  3. Critically evaluate strategies to help people live well with a long term condition.
  4. Critically appraise and evaluate current health care policy, and local and national guidelines, in relation to long term conditions.
  5. Using critical insight, demonstrate the knowledge and skills required to manage complex care in those with long term conditions.


Assessment

  1. A 20 minute presentation of a case study from practice of a patient with a long term condition.
  2. A critical reflection and practice evaluation of strategies used to help a patient with long term condition/s, including references to local and national health care policy and guidelines.
