Course overview
This module will provide the opportunity for students to synthesise the knowledge and skills required to work effectively with those living with a long term condition, and the management strategies that can be employed at each stage of the patient journey.
Indicative Content
- Long term conditions in context.
- Pathophysiology, symptomology, care and management of a variety of long term conditions e.g. respiratory disease, cardiac disease, diabetes, dementia, frailty.
- Psychosocial impact of living with a long term condition. Self-management in long term conditions.
- Primary and secondary prevention Managing complex care
- Long term conditions and mental health.
- Empowerment
- End of life care and Palliative care, LPA, DNAR’s and advance care planning
- Ethical considerations in long term conditions
- Multi morbidity, and poly pharmacy and prescribing decisions
- Structured medication reviews
Learning Outcomes
- Critically apply knowledge and understanding of the pathophysiology, symptomology, care and management of long term conditions.
- Synthesise evidence related to the physical, social and psychological impact of living with a long term condition.
- Critically evaluate strategies to help people live well with a long term condition.
- Critically appraise and evaluate current health care policy, and local and national guidelines, in relation to long term conditions.
- Using critical insight, demonstrate the knowledge and skills required to manage complex care in those with long term conditions.
Assessment
- A 20 minute presentation of a case study from practice of a patient with a long term condition.
- A critical reflection and practice evaluation of strategies used to help a patient with long term condition/s, including references to local and national health care policy and guidelines.
