Course overview

This module is for registered practitioners working in a clinical practice setting working with, and managing, the care of patients/clients with long term conditions.

The module will focus on a variety of long term conditions that may be encountered in practice, together with the impact that these, and their management, may have on those living with them on a daily basis. It will explore the progression of long term conditions from diagnosis to end of life, the complexity of multi-morbidities, and the leadership and management strategies that can be employed to enable effective care provision and support. This will all be set within the long term conditions current policy landscape, guidelines and evidence.

This module will enable you to synthesise the knowledge and skills required to work effectively with those living with a long term condition, and the management strategies that can be employed at each stage of the patient/client journey to meet their diverse needs.

Upon successful completion of this module as a standalone entity, credit gained could be utilised as part of the PGDip Enhanced Practice.


Learning Outcomes

To critically apply knowledge and understanding of the pathophysiology, symptomology, care and management of long term conditions

To synthesise evidence related to the physical, social and psychological impact of living with a long term condition.

To critically evaluate strategies to help people live well with a long term condition

To critically appraise and evaluate current health care policy, and local and national guidelines, in relation to long term conditions

Using critical insight, demonstrate the knowledge and skills required to manage complex care in those with long term conditions.


Assessment

1. Development of a poster based on a long term condition

2. A 3000 word case study based on the complex care management of a person with a long term condition
