Course overview

This module will provide the opportunity for students to synthesise the knowledge and skills required  to work effectively with those living with a long term condition, and the management strategies that  can be employed at each stage of the patient journey.

Indicative Content 

  • Long term conditions in context.
  • Pathophysiology, symptomology, care and management of a variety of long term conditions e.g. respiratory disease, cardiac disease, diabetes, dementia, frailty.
  • Psychosocial impact of living with a long term condition. Self-management in long term conditions.
  • Primary and secondary prevention Managing complex care
  • Long term conditions and mental health.
  • Empowerment
  • End of life care and Palliative care, LPA, DNAR’s and advance care planning
  • Ethical considerations in long term conditions
  • Multi morbidity, and poly pharmacy and prescribing decisions
  • Structured medication reviews
