Course overview

This module is for registered practitioners working in a clinical practice setting where they work with patients /clients living with long term conditions.

The module will focus on a variety of long term conditions that may be encountered in practice, together with the impact that these may have on those living with them on a daily basis. It will explore the challenges of living with a long term condition from diagnosis to end of life, and the care and support needs of those undertaking this journey. This will all be set within the long term conditions current policy landscape, guidelines and evidence.

This module will enable you to develop the knowledge and skills required to work effectively with those living with a long term condition, and to meet their diverse needs.
